Check the IP assigned to the device:
[✓] IPv6 address(es) bound to the eth0 interface:
2003:c8:df0f:3300:ceb3:69a3:740e:6001 does not match the IP found in /etc/pihole/setupVars.conf
(Use IPv6 ULA addresses for Pi-hole)
fe80::bd13:c13b:421d:ab37 does not match the IP found in /etc/pihole/setupVars.conf
(Use IPv6 ULA addresses for Pi-hole)
^ Please note that you may have more than one IP address listed.
As long as one of them is green, and it matches what is in /etc/pihole/setupVars.conf, there is no need for concern.
The link to the FAQ is for an issue that sometimes occurs when the IPv6 address changes, which is why we check for it.
And:
*** [ DIAGNOSING ]: Name resolution (IPv6) using a random blocked domain and a known ad-serving domain
[✓] monetizer101.com is :: via localhost (::1)
[✗] Failed to resolve monetizer101.com via Pi-hole (2003:c8:df0d:8a00:366a:9023:8a06:d037)
[✓] doubleclick.com is 2a00:1450:4001:81b::200e via a remote, public DNS server (2001:4860:4860::8888)
Then check /etc/hosts
[✗] Nov 12 11:28:52 dnsmasq[14906]: bad address at /etc/hosts line 1 (Why do I see "bad address at" in pihole.log?)
Lastly check your Apache modifications as they do not appear to be correct.
