You have a few configuration errors.

As long as one of them is green, and it matches what is in /etc/pihole/setupVars.conf, there is no need for concern.
The link to the FAQ is for an issue that sometimes occurs when the IPv6 address changes, which is why we check for it.

Please run a pihole -r
and select reconfgure.

This is a known bug and will be fixed in the next release, should be today or tomorrow. IN the mean time you can edit /etc/lighttpd/lighttpd.conf
and change pihole/index.php
to /pihole/index.php
